What our inpatient service covers
Nutrition screening within 24 hours of referral
Using validated tools: MNA-SF, NRS-2002 and GLIM criteria for malnutrition diagnosis.
A personalised nutrition prescription
Energy, protein, fluid, electrolyte and micronutrient targets matched to your diagnosis and phase of illness, including oral nutrition support, enteral feeding and parenteral nutrition, advised together with your medical team.
Daily ward rounds
We re-assess and adjust the plan based on how you’re responding, every day, including weekends.
Discharge planning
A clear post-discharge nutrition plan, with your outpatient follow-up booked before you leave the ward.

If a Reset Health dietitian saw you during your hospital stay, your outpatient follow-up is with the same team. We have your records, your hospital diagnosis codes, and the full context of your admission.
This continuity matters most for:
- Cancer patients transitioning from acute admission to ongoing treatment
- Surgical patients needing ERAS-protocol pre-op preparation and post-op recovery
- Renal patients new to dialysis or post-transplant
- Diabetes patients stabilising after an admission for hyperglycaemia or DKA

How hospital referrals work
